حاجات اساسية في بداية كل اكزامنيشن سواء جراحه او باطنه : 
1- WIPPPER مهمه#
Wash your hand , Interduce yourself , pression,Position of Pt. , privacy , Exposure, Right Side of the Pt. 
- - - 
*احيانا يضيفو عليها Nurse present & Good light source 
* ناس نسيو (وبير ) دي وفشلو في الاختبار ومهم تكون هادي من البدايه وما تتوتر عشان تتزكرها ! 
* ممكن أحيانا يحطلك سرير المريض عل اساس تتضطر تجي من اليسار ؛ هنا لازم تقول للدكتور المختبر انو انا المفروض اوقف يمين المريض عشان يعرف انك منتبه 
* ال position يختلف من اكزام للتاني ..

2 - General Examni. (ABCDEF) مهمه #
A=Appearance , B=Body built , C=Colour and consciousness , D=Deformaty or Disability , E=environment , F=vital signs (pulse +Temp.+ Resp. Rate +Blood pressure +O2 concentration)

* كل الGeneral بدون ما تلمس المريض ب بعينك معادا ال F ؛ ومهم تعرفي ووتتمرني كيف تسويها الخمسه / النبض وقياس الضغط الخ ..
* حسب الدكتور غالبا رح يخلوك تسوي وتقول البدايه ويقولك Skip الباقي بس لازم تكون حافظها ..

بعد كدا تبدا حسب الSystem او الExamination اللي يطلبو منك الدكتور ؛ لكل اكزامنيشن حطتلك رابط فيديو فيه شرح بالطريقه ؛ ورح تلاحظ في كل فيديو رح يبدأو باول نقطتين لانها مهمه في الاختبارات .. و في الاختبارات رح تمشي بالتسلسل دا : 
-Inspection-Palpation-Percussion -Auscultation
* مهم جدا انك تشرح للمريض وانك تفهمه انتا كل شي بتعمله ليه وتاخد ازنه وفي الpalpation ال Eye contact مهم ويركزو عليه الدكاتره ..

  • SURGERY
Topics included in OSCE exam
A-history component:
1-goiter/neck swelling: hyper-hypothyroidism symptoms,lump history,constitutional symptoms,history of radiation exposure,family history,pressure symptoms (dysphagia,dyspnoea,hoarseness) ,pain history ,history of trauma,skin changes....etc.
2-breast mass/pain: pain history,history of trauma, lump history,constitutional symptoms,relation to period,breast ca risk factors eg.menarche & menopause age,age at first childbirth (>or< 32y.o),hormone replacement therapy use,breast feeding,family history ,nulliparity,nipple discharge,skin changes, previous breast ca or breast surgery in either breast...etc.
3-upper/lower GI bleed: blood characteristics, associated symptoms, bleeding tendency and drugs inducing bleeding, previous procedures hospitalization and blood transfusion ,trauma history...etc
4-acute abdomen (upper eg. acute cholecystitis and pancreatitis, lower eg. acute appendicitis, diffuse eg. bowel obstruction, or midline eg. bowel ischemia): pain questions, associated symptoms eg.vomiting,nausea,anorexia,change in bowel habit, constitutional symptoms ,relation to food ,abdominal mass or distension, lump history if present, previous similar history and past surgical history, trauma history ....etc.
5-obstructive jaundice: pruritis, dark urine, pale stool, yellowish discoloration of skin & sclera + abdominal pain & GI system review
6-anorectal conditions (eg.hemorrhoids, fistula and fissure in ano, perianal abscess, pilonidal sinus/abscess,rectal prolapse and tumors...etc.): history of anal or perianal pain,discharge,PR bleeding,mucus,swelling, constitutional symptoms, protruding mass,itchiness,tenesmus ,lump history, trauma history ...etc.
7-surgical soft tissue infections (eg.cellulitis,abscess ,diabetic septic foot ...etc.) :pain history,fever ,swelling,discharge ,discoloration ,history of trauma,lump and/or ulcer history....etc.
8-hernia (eg. inguinal,femoral,paraumbilical,incisional ....etc) :pain history, lump history , precipitating factors (eg. chronic cough,chronic constipation,chronic dysuria,previous abdominal surgery especially with wound infection,conditions increasing intra-abdominal pressure like ascites or tight clothes or occupations with lifting of heavy objects,multiparity,history of complications (eg. bowel obstruction and hospitalization for that) associated symptoms, skin changes,previous surgery at same site (recurrent hernia) ...etc.
9-abdominal mass: lump history,associated symptoms,GI & GU system review ...etc.
B-physical exam component:
1-Neck/thyroid exam
2-breast exam
3-abdominal exam
4-general exam including thyrotoxicosis hand and eye signs mainly
5-ulcer and lower limb exam
6-lump exam

C-Data show component includes: pictures ( similar to the ones in browse) and radiology like chest and abdominal X-rays plain or with contrast (eg.barium swallow ,meal ,follow through or enema) , mammogram, ultrasound or CT abdomen and pelvis , diagrams/illustrations, and common instruments/tubes in surgical practice including nasogastric tube, chest tube with underwater seal, foley catheter ,T tube and central line all with scenarios and questions

#نصايح .. 
1- في دا النوع من الاختبارات المزاكرة الجماعيه افضل شي يثبت المعلومات ويعلمك سواء كنت انسان تتعلم بطريقه سمعية او بصريه ؛ شوفي اشخاص يناسبهم ويناسبك طريقه مزاكرتهم ؛ شوفو المعلومات والفديوهات سوا اكتبوها في ورق خارجي ك خطوات ؛ طبقو وعلمو بعض لانه كل واحد بينتبه وبيركز مع التاني اش نقص واش سوا بطريقه صح او غلط ؛ سوو جو زي الاختبار بالوقت مثلا 5 دقايق وشخص ممتحن والتاني الطالب وواحد يشوف اش سويت وايش ما سويت ويديك درجه ؛ تقيم نفسك ومرة عل مرة بتتعلمي .. 
2 - لازم تعرفو كل حركة بتسوها ليه بتسوها يعني بسوي كده عشان اشوف االشي الفلاني لانه ممكن يسالكم فيها ؛ يقولك ليه بتسوي ال Test دا مثلا فلازم تكوم فاهم مو بس حافظ ! 
3 - لازم يكون عندك معلومات نظرية عن كل موضوعات الاختبار سواء ميدسن ولا سيرجري ؛ عشان لو سالك تقدر تجاوب ايش ال DDx حقت اللي بتعملو دا ومهم اسماء كل sign وحقت ايه كيف بتسويها ..
4 - التكرار مهم الأهم انك تتقن ال technique 
5- كتاب ال ABC اللي سووه طلاب كليتنا الله يعطيهم العافيه حاجة كويسه ووممكن يفيدك كتير ..
